what are home care packages?

Home Care Packages (HCP) are one of the ways that older Australians can access affordable care services to get some help at home. They are designed for those with more complex care needs.

Home Care Packages can be an option if you need a coordinated approach to the delivery of your help at home - perhaps because you need help with many everyday tasks, or the care you need is more complex or intensive.

HOW Neuro junction HELP ME?

Neuro Junction work with home care package providers to deliver services such as physiotherapy or occupational therapy for clients to help their physical capabilities and assist them in increasing their independence with activities of daily living. Our therapists work with case managers to ensure that they are updated with a client’s current treatment plan and any future management recommendations.

PHYSIOTHERAPY

Neuro Junction offer physiotherapy services to clients with home care packages. Our physiotherapists will work with you to improve your physical function. Such as following orthopaedic surgery, a hospital admission or after a fall. Physiotherapists can help you improve your balance and falls prevention, increasing endurance and fitness levels and helping increase your strength.

OCCUPATIONAL THERAPY

Our occupational therapists provide services to clients with a Home Care Packages such as functional capacity assessments, home modifications, support in applying for assistive technology such as walking aides and dressing aides and helping clients become more independent in everyday tasks.

Hydrotherapy

Neuro Junction offers hydrotherapy programs for clients with a Home Care Package and can provide training and support for carers to assist in these programs.
